Share your experiences of hospital discharge in the last year

We would like to hear from our Croydon Asian, African and Caribbean communities who have been discharged from an NHS hospital in England in the last 12 months and experienced delays or difficulties. Or are you a carer, relative or friend, supporting someone who has?

We would like to know if:

The discharge was delayed by at least a day
The discharge happened before you felt ready
Care, support, equipment, medication, transport or follow-up was not in place
Information was unclear
