Transforming Primary Care for London – Proactive Event, 13th April
Transforming Primary Care for London – A Strategic Commissioning Framework’, published in March 2015, provides a new vision for general practice across the capital, and an overview of the considerations required to achieve it.
An essential component of the framework is Proactive Care. London needs a primary care service that can systematically enable patients to self-care, provide behavioural change support and/or refer patients to those who can assist with improving health and wellness for all. General practice is the most accessed part of the health system and is therefore uniquely placed to become the hub for proactive care in the future.
The Proactive Care event will provide ample opportunity to network with key stakeholders in primary care whilst also learning and share best practices across London.
